I had that situation and managed to get it fixed without contacting the isp by accessing the modem/router settings and port forward 32400 (or whatever you have your Plex set to) with the internal Ip of my actual router. The actual router would have the port 32400 forwarded with the internal ip of the server running the machine.I have had the plex server for a while, I currently have it setup at home while I'm at university. The issue is that I don't have the ability to stream the videos from there in HD only 480p. I know it is not due to the network speed as I can play them In 1080p if I VPN onto the network.

I had IPv4 ports open to plex and plex was happy. My new router recent got an IPv6 and v4 address. Plex would go out look at its IP, it would see its v4 and be happy.
